DESCRIPTION of NEEDED SERVICES: Individual is seeking a Voc Hab provider for Monday through Thursday services. The individual would like to continue developing his job skills and working towards a future in Community Integrated Employment. He would need assistance with learning new job skills through demonstration and repetition.

DESCRIPTION of INDIVIDUAL: Individual is 23 and lives at home with family. He is able to communicate verbally and understand communication and is fully ambulatory. He is independent with most tasks once he has attempted them a few times.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Autism, ADHD
HOUSING: With family
DAY PROGRAM: Currently in another program, seeking other.
SUPPORTS NEEDED: VH, transportation
