DESCRIPTION of INDIVIDUAL: 17 years old, enjoys going out in the community. His cousin and family are very important to him. He has a close connection with his family dog, Chase. He enjoys having his cell phone and communicating with family and friends. He is a joy to be around. He enjoys playing games, having bonfires, and building Legos. He benefits from verbal reassurance, giving space and having a quiet space.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Diagnosed with Duchenne Muscular Dystrophy, Sleep Apnea, Oppositional Defiant Disorder and Anxiety.

DAY PROGRAM:
SUPPORTS NEEDED: Lives in family home. Assist with community integration, attending rec events and interested activities. Assistance with ADL’s, transportation/transitioning to and from wheelchair. Use of Hoyer lift, Days/hours are flexible depending on scheduled activities.
