DESCRIPTION of NEEDED SERVICES: This individual is looking for a male HPC provider. Days and times to be discussed with the family during the interview process.

DESCRIPTION of INDIVIDUAL: 22-year-old male, loves playing with his toys (cause and effect), listening to music, hanging out with his family/friends.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting; Medication Administration Cert 1, utilize a ceiling lift (if waking up and putting the individual into bed).
DIAGNOSES: Cerebral Palsy, developmentally delayed, Seizures.
HOUSING: Individual lives with his mother in Mentor, Ohio.
DAY PROGRAM: None
SUPPORTS NEEDED: Individual needs assistance with all aspects of daily living; hygiene, dressing, bathroom, meal prep, transportation, medication, etc.
