DESCRIPTION OF REQUESTED SERVICES: 21-year-old male seeking looking for a new Adult Day support provider to attend a program 5 days per week Monday-Friday. Individual would need assistance navigating and following a visual schedule for him to navigate through his day at program.

DESCRIPTION OF INDIVIDUAL: Individual communication via speech generating device, and gestural prompts. He enjoys spending time on the swing, playing with various sensory toys, and following a routine. He works best with a consistent routine, receiving simple and direct language from staff, and having a quiet and calm space to go to when he is upset or not feeling well. Individual does have a history of physical aggression, wandering from staff supervision, and self-injurious actions towards staff and peers. Individual has not wandered from staff or become aggressive towards peers in approximately 2-3 years.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. It is recommended that a provider has training in a crisis intervention curriculum for supports focusing on de-escalation.
TRANSPORTATION NEEDS: Individual requires a non-modified vehicle
DIAGNOSES: Individual is diagnosed with Autism, Anxiety disorder, and sensory processing disorder. He is allergic to Ketamine and has seasonal allergies
HOUSING: Individual resides with mother and father in Concord
DAY PROGRAM/EMPLOYMENT: Individual currently does not attend an ADS program mat this time.
