DESCRIPTION of INDIVIDUAL: Individual is a happy boy who is mobile. He requires 24-hour supervision, as he is an elope risk. He needs help with all ADL’s.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Tetrasomy 5p, Developmental Delay, Depth Perception issues, Seizure Disorder
HOUSING: Lives with parents in a house in Mentor, Ohio
SUPPORTS NEEDED: Assistance with ADL’s and visual supervision
