DESCRIPTION of INDIVIDUAL: Individual is 23-year-old female who thrives on routine and patience of others. Loves to take walks in the neighborhood, use her iPad, computer, video games, animatronics, and Animae movies. She is able to make decisions about what she wants to do but can be impatient with others and things that don’t interest her, sometimes speaking whatever is on her mind.
PROVIDER REQUIREMENTS: PD-HPC certification
DIAGNOSES: Autism, Mitochondrial Dysfunction, Asthma, Low Tone Disorder, Seizure Disorder (Epilepsy), Mild Cerebral Palsy, and Hypotonia.
HOUSING: Lives in Mentor on the Lake with her family.
DAY PROGRAM: None
SUPPORTS NEEDED: Follow seizure protocol- Individual can typically tell if she is going to have a seizure. Needs social support to communicate with others appropriately in the community. Does best following her routine and knowing what to expect. Provide Auditory supervision at home/ Auditory and Visual supervision in the community. Help to transition her to new surroundings until it becomes part of her routine. Assist to hold/open items as needed and encourage a gluten-free diet.
