DESCRIPTION OF REQUESTED SERVICES: Individual is looking for an NMT driver Monday through Friday for pick up and drop off. She would need to be picked up in Perry PRIOR to 8:00 am and dropped off in Mentor. Due to household schedules, the pick-up time is inflexible. In the afternoon she would need to be picked up in Mentor by 2:00 pm and taken home to Perry; approximately 19 miles one way.

DESCRIPTION OF INDIVIDUAL: Individual is a 28-year-old female. She is non-verbal (vocabulary less than 50 words) but can communicate with gestures or through icon boards. She loves Veggie Tale books and movies. Having Veggie Tale books in her purse/bag is very important to her and enjoys when someone reads to her. Individual needs time to process her thoughts and to be given choices. She likes to be around people and watch people. She can be hesitant at first to join in activities, but over time, she will participate with assistance from her support person. She participates in Miracle League and Metropark Adaptive Sports.
PROVIDER REQUIREMENTS: Provider must adhere to the transportation schedule to meet individual’s timeframe needs.
TRANSPORTATION NEEDS: Provide NMT, per trip, in a non-modified vehicle.
DIAGNOSES: Individual is diagnosed with Down’s Syndrome, Hypothyroidism, OCD, Tourette’s, ASD, and ADHD.
HOUSING: Individual lives in Perry with her mom. She visits her father out of state several times a year; during which, NMT services would not be warranted.
DAY PROGRAM/EMPLOYMENT: Attends C& J Provider Services in Mentor.
SUPPORTS NEEDED: Individual requires visual supervision in the community as she will wander if she’s not sure what she should be doing. She is independent in her ambulation but occasionally likes to hold someone’s hand for comfort. Individual needs provider to point out a tripping hazard or a step/curb and give a verbal prompt as she doesn’t consistently wear her glasses and has difficulty with depth perception. She is less than 5 feet tall and will need a running board or stepstool if the NMT vehicle has a high H-Point.
