DESCRIPTION of NEEDED SERVICES: This individual is looking for an HPC provider at least 5 days a week, from 3p-7 or 8:00p.

DESCRIPTION of INDIVIDUAL: The individual is 7 years old. She loves to watch TV, movies, iPad, and her phone. She also loves to swing.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting; HPC transportation, Med Cert 1.
DIAGNOSES: Cerebral Palsy, Autism, Seizures.
HOUSING: Individual lives with her family in Mentor, Ohio.
DAY PROGRAM: None
SUPPORTS NEEDED: The individual needs total assistance with hygiene, getting dressed, meal preparation, toileting, transportation. Assistance up and down stairs, uneven terrain, medication administration, etc.
