DESCRIPTION OF INDIVIDUAL: Active fun female in late 30s that likes to be involved in a variety of activity and socialization opportunities. Needs someone to hang out with to give parents a break- staffing with get-up-and-go attitude and to encourage a healthy lifestyle. Needs support in times of increased anxiety and assistance with minimal ADLs. Loves animals- especially dogs, bike riding, bocce, music, and dancing.
PROVIDER REQUIREMENTS: Medication administration certification benefit but not necessary. Residential Respite, HPC-T certified.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: ID, PDD, Anxiety, ADHD, PMDD
HOUSING: Lives with Parents in Mentor
DAY PROGRAM/EMPLOYMENT: Day Program 5x weekly
