DESCRIPTION OF INDIVIDUAL: Bright 30-year-old male who is looking for Residential Respite services in his home or in the community to give parents a break. Loves attention. Has a great sense of humor and likes to joke around but is respectful. He loves swimming, ice skating, playing baseball, playing on the computer/Nintendo Switch and just hanging out. He enjoys playing a lot of different computer games but also likes to go out to have some fun. His favorite restaurant is Applebee’s; usual order is grilled cheese but will switch it up sometimes! He enjoys keeping a calendar to check off days and likes to talk about the traveling that he has done with his family. Great guy to hang out with!
PROVIDER REQUIREMENTS: Medication administration certification benefit but not necessary. Residential Respite, HPC-T certified.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Smith Magenis Syndrome, seizure disorder, seasonal allergies, and iron deficiency
HOUSING: Lives with Parents in Painesville near Riverside HS.
DAY PROGRAM/EMPLOYMENT: Day Program 5x weekly
