DESCRIPTION OF REQUESTED SERVICES: 75-year-old male looking for a money management provider. Needs representative payee for JFS as well as Authorized Representative for Social Security.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Prefers agency.

DESCRIPTION OF INDIVIDUAL: Individual is very friendly, kind and independent. He can make small purchases with verbal assistance from provider, and needs total assistance with paying bills, and maintaining SNAP benefits, Medicaid, and Social Security.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at ISP meeting.
TRANSPORTATION NEEDS:
DIAGNOSES: Autism
HOUSING: Individual lives in an apartment by himself, with drop-in services from an HPC provider 3-4 days a week.
DAY PROGRAM/EMPLOYMENT: He works in the community 4 days a week.
