DESCRIPTION OF REQUESTED SERVICES: 25-year-old male who resides in Willoughby with his parents looking for ADS 5 days a week to connect with others in the community. Will need to NMT to and from day program. Need assistance with expressing wants/needs as he won’t always express if he is sick, etc) assistance with unfamiliar routines/environments, assistance with safety when in the community, especially parking lots, crossing the street, money.

DESCRIPTION OF INDIVIDUAL: Friendly male who enjoys crafts, rec outings, movies, Disney songs, outdoors. He does not like loud noises, dancing. Likes to use the same door to go in and out of his home. Currently engages in self-stimming behavior and occurrence of property destruction and aggression at home; however, this behavior has not occurred outside the home.

DIAGNOSES: Autism, Mild Intellectual Disability
DAY PROGRAM/EMPLOYMENT: Currently not attending ADS, looking to start first ADS program.
