DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 19-year-old male who is very friendly. He resides with his mother in Willoughby and is looking for an HPC provider for specifically through 2/16/26-2/27/26 from 5a-8a and then again 3p-4p. No alone time.
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: ADHD/ Moderate ID
HOUSING: He resides with his family in Willoughby.
DAY PROGRAM: Yes
