DESCRIPTION of NEEDED SERVICES: Individual is looking for an NMT driver Monday through Friday for pick up and drop off starting Thursday, December 18th, 2025. He would need to be picked up in Painesville by 7:30am and dropped off in Willoughby (PACKIJ). In the afternoon he would need to be picked up in Willoughby by 1:00p and taken home to Painesville. Approximately 15 miles one way.

DESCRIPTION of INDIVIDUAL: Individual is a 21-year-old male. He is verbal and enjoys socializing. He enjoys trains, playing basketball, playing golf, riding in the car, listening to music, watching videos, etc.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Fetal Alcohol Syndrome, ADHD, Moderate Intellectual Disability, Motor Dyspraxia, Tourette’s Disorder, Tic Disorder, Language Disorder, Disruptive Mood Dysregulation Disorder, Generalized Anxiety Disorder, OCD, Insomnia, Constipation, Iron and Vitamin D3 deficiency and Asthma.
HOUSING: Currently he lives on his own (with 24/7 staffing) in Painesville, Ohio.
DAY PROGRAM: Will be attending PACKIJ Solutions M-F.
SUPPORTS NEEDED: The individual needs auditory supervision in his home and visual supervision while in the community. He needs verbal reassurance and redirection, as he may overwhelm himself sometimes.
