DESCRIPTION OF REQUESTED SERVICES: Seeking HPC provider to assist with community access and activities, ideally in a similar age range. Primarily needs assistance with transportation and making purchases, may need help with pedestrian safety/navigation if overwhelmed. Seeking 5+ hours/week. Her schedule would allow weekends mostly, but she would like to work out a monthly calendar ahead of time in case any events pop up that she wants to go to on other days. Weekdays only allow about 3 hours of time due to work schedule.

DESCRIPTION OF INDIVIDUAL: Individual is 33 years of age, very independent, fully verbal and ambulatory. She works M-F. She is a strong self-advocate and a strong sense of what she wants to do.
PROVIDER REQUIREMENTS: HPC and HPC transportation certification. Female staff of similar age preferred.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: TBI, Asthma
HOUSING: Lives in Willoughby, open to activities inside and outside of Lake County.
DAY PROGRAM/EMPLOYMENT: Employed.
