DESCRIPTION OF REQUESTED SERVICES: This individual is a 50-year-old male who resides in Concord. He is looking for Residential Respite services in the community while his guardian is recovering from a medical procedure. This individual requires physical to total assistance with hygiene and ADL’s and auditory to visual supervision.

DESCRIPTION OF INDIVIDUAL: This individual requires auditory supervision in the home and visual supervision in the community. He uses a very limited number of words and phrases to communicate and will laugh when he is unsure how to respond. He has a history of smacking windows or surfaces paired with cursing at others when he is in a heightened state of anxiety but is easily redirected when given a quiet place to decompress. He enjoys dancing, watching TV, looking out the window and listening to music.
PROVIDER REQUIREMENTS: Med. cert. 1
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Moderate ID and seizure disorder (has not had a seizure in decades and is managed with medication)
HOUSING: This individual lives at home with his mother.
DAY PROGRAM/EMPLOYMENT: C&J day program.
