DESCRIPTION OF INDIVIDUAL: Individual is an 18-year-old male, he is verbal and has no mobility concerns. Individual is seeking a provider certified in HPC to provide assistance to him in a new home, ideally a congregate setting. Individual is independent in most ADLs, he requires frequent verbal reminders for completion of tasks and assisting creating and maintaining a daily schedule. Individual requires med certified staff for oral medication only.
PROVIDER REQUIREMENTS: All DODD requirements for waiver provider.
TRANSPORTATION NEEDS: N/A
DIAGNOSES: Mild ID (FSIQ- 69), ADHD, Reactive Attachment d/o, PTSD, Insomnia, Intermittent Explosive D/O, Autism Spectrum Disorder and Amplified Musculoskeletal Pain Syndrome (AMPS), Auditory Processing D/O, Dyspraxia, Language D/O, and Specific Learning Disability in Reading Comprehension, Mathematics (Dyscalculia), and Writing (Dysgraphia)
HOUSING: Individual resides in a single-family home with his parents in Willoughby but is seeking a new home with roommates in Lake County.
DAY PROGRAM/EMPLOYMENT: Individual has graduated high school and is currently exploring post-secondary options and day programming.
