DESCRIPTION OF REQUESTED SERVICES: 73-year-old female looking for a wheelchair accessible congregate setting. Individual has left sided weakness and is unable to transfer independently. Individual would need assistance with meals (cutting food), dressing, bathing, transportation.

DESCRIPTION OF INDIVIDUAL: Individual needs assistance with hygiene tasks (bathing, toileting, cutting up food) due to left sided weakness. She uses a wheelchair when out in the community or day program but will use a walker while at home. She requires assistance to due balance. She requires 24/7 supervision due to mobility and assistance with transferring.
PROVIDER REQUIREMENTS Individual can self-administer medications.
TRANSPORTATION NEEDS: Non-modified
DIAGNOSES: Individual is diagnosed with left sided Paralysis epilepsy, grand mal seizures, Mild Intellectual Disability. She wears a brace on her left foot. She has a VNS located on her right side. She carries her VNS magnet in her bag that she takes with her to the day program.
HOUSING: Looking for Congregate Setting.
