DESCRIPTION OF INDIVIDUAL: Individual is a 15-year-old female who is very friendly but struggles with aggression and property destruction. She enjoys playing with her toys, books and listening to music or watching TV. She needs total assistance with all hygiene related tasks, physical assistance in the bathroom (BM/Menstrual), close and constant supervision in the community and auditory in the home, and total assistance with meal prep.
PROVIDER REQUIREMENTS: Med cert 1.
TRANSPORTATION NEEDS: Non-Modified
DIAGNOSES: Autism, OCD and Anxiety
HOUSING: At her home in Eastake.
DAY PROGRAM/EMPLOYMENT: Individual goes to school during the week.
