Individual is very outgoing and enjoying joking around. Individual can become frustrated and become verbal aggressive towards staff and peers.
PROVIDER REQUIREMENTS: HPC
TRANSPORTATION NEEDS: Requires transportation to and from the community.
DIAGNOSES: Mild ID, Carpal Tunnel, Arthritis, Depression, Bipolar DO; OCD; Pre-Diabetes, Hypertension, blindness.
HOUSING: Lives in residential group home in Madison
DAY PROGRAM/EMPLOYMENT: Attends voc program, NMT provided by provider.
