DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 22-year-old male with a good sense of humor. He resides with his parents in Mentor and works at Breckenridge Rehabilitation. Individual has difficulty with social cues and recognizing non-verbal communication of others. Individual requires a consistent schedule with one specific staff. A male staff is required. This young man needs a mentor, someone with patients and follow through. Individual drives himself to and from work and the community. He can drive but also enjoys riding with others. He loves the YMCA and working out. Young man loves pinball and enjoys league play in Cleveland. Provider would need to encourage expressive communication and assist with receptive communication. Individual may need assistance regulating emotions due to the behavior of others. A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Would like to have a younger male staff.

HOUSING: Resides with his mom and dad in Willoughby
DAY PROGRAM: Community Employed
SUPPORTS NEEDED: FPC in community, FTN, supervision in community
