DESCRIPTION OF INDIVIDUAL: Individual is a 22 y/o male. He has limited verbal skills; he understands English and typically expresses himself through scripts, some basic American Sign Language, facial expressions, gestures, or through the LAMP app (84 icons) on his tablet. He thrives with a visual schedule and prefers to adhere to that schedule once it is established. The individual currently resides with his family in Willoughby.
The individual is typically very easy-going and gentle, seeking out affection from his trusted adults. If he is startled by a loud, unexpected noise, if there is a sudden unexpected change, or if he feels frustrated that someone doesn’t understand him, then he can become dysregulated and exhibit self-injurious behaviors (i.e. biting his own hand without breaking skin, pushing on his own forehead, pushing on his eye, slapping himself). However, he utilizes regulation skills and returns to baseline. His most used regulation skills are gross motor breaks (e.g. wall push-ups, walks, lifting something heavy, riding a stationary bicycle) but he also enjoys regulating with a weighted blanket while watching movie clips on YouTube. He has many interests, such as being outside, going for walks, spinning with dance ribbons, riding his bicycle, spending time in the water, playing baseball, and watching movies.
PROVIDER REQUIREMENTS: N/A
TRANSPORTATION NEEDS: The family plans to transport the individual for the first several months until the individual becomes habituated to the new routine and location. He may utilize NMT services in a non-modified vehicle in the future.
DIAGNOSES: The individual is diagnosed with Epilepsy, Autism, and Intellectual Disability/Developmental Delay.
HOUSING: Lives at home with his family.
DAY PROGRAM/EMPLOYMENT: Looking for a program 5x week.
