DESCRIPTION OF REQUESTED SERVICES: 46 y/o male looking for unscheduled HPC Transportation in a modified vehicle each month to transport him to various social/medical/personal appointments in the community.

DESCRIPTION OF INDIVIDUAL: Individual is a 46 y/o male who utilizes a manual wheelchair. Individual doesn’t have an adapted vehicle of his own and is not comfortable utilizing lift options to place him into a non-modified vehicle. He is specifically looking for an individual or agency provider with a modified vehicle who could provide HPC transportation on an unscheduled basis. The individual doesn’t have any behavior concerns.
PROVIDER REQUIREMENTS: Must have a modified vehicle.
TRANSPORTATION NEEDS: Modified vehicle only.
DIAGNOSES: Spina Bifida and Congenital Hydrocephalus
HOUSING: Individual resides with his Shared Living provider in Mentor.
DAY PROGRAM/EMPLOYMENT: None
