Female in her 50’s, is interested in moving into a congregate setting with HPC supports for most ADL’s. She currently attends ADS M-F. She enjoys attending community activities.

DESCRIPTION OF INDIVIDUAL: May display OCD at times, becomes fixated on something and may become verbally aggressive, she can be redirected. She has an uneven gait and wears a gait belt for support but is independently mobile.
PROVIDER REQUIREMENTS: Medical administration certification 1, unable to self-administer.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Moderate ID, CP, seizure disorder, impulse control disorder, bipolar disorder
HOUSING: Currently lives in congregate setting
DAY PROGRAM/EMPLOYMENT: ADS Monday – Friday
