DESCRIPTION OF REQUESTED SERVICES:
Looking for HPC provider to assist with morning and afternoon routines for a 16-year-old female. Provider to help with getting dressed, finishing meals, and with getting on and off the school bus.

DESCRIPTION OF INDIVIDUAL: Individual is a 16-year-old female. She attends school during the day but is in need of assistance before and after school. She is nonverbal, but smiles, laughs, and is generally always happy. She understands a lot of what is said to her. She needs total assistance with all ADLs and has a wheelchair. She loves watching movies and being around people. She is very sociable! She enjoys music and having fun!
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Modified transportation- wheelchair accessible
DIAGNOSES: Rett Syndrome
HOUSING: Individual lives with mom in Fairport Harbor.
DAY PROGRAM/EMPLOYMENT: Individual attends school M-F in Painesville.
