DESCRIPTION OF REQUESTED SERVICES:
Individual is looking for an adult day program/voc hab, NMT provider 5 days/week (M-F). She requires assistance with managing her anxiety (encourage breathing exercises), using calm voice, assist with making purchases in community, physical assistance to cross the street using gait belt, getting on/off transportation and curbs/uneven surfaces.

DESCRIPTION OF INDIVIDUAL: Caring, friendly and enjoys talking with others, fun to be around. She has OCD tendencies, at times can become verbally aggressive, provide verbal prompts in calm demeanor to re-direct.
PROVIDER REQUIREMENTS: Medical administration certification 1, unable to self-administer.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Moderate ID, CP, seizure disorder, impulse control disorder, bipolar disorder
DAY PROGRAM/EMPLOYMENT: ADS Monday – Friday
