DESCRIPTION OF REQUESTED SERVICES: 23-year-old female individual seeking a congregate setting. Prefers to live with one other female but is open to options. She is currently residing in an ICF and needs support to continue to increase her independent living skills in the community. Support is needed to budget money, find social opportunities, attend medical appointments, etc. Team does not believe that she needs 24/7 supports and she can manage time alone (hours to be determined).
SEEKING AGENCY OR INDEPENDENT PROVIDER: Looking for agency with all female congregate home available.

DESCRIPTION OF INDIVIDUAL: This individual is kind, caring, and motivated. She maintains community employment with supports of a job coach and is always setting goals for herself. She has many daily living skills but has not had the chance to develop all skills due to her current setting. She is excited to move to a community location, learn supports to access Laketran, look into classes that suit her career interests, learn to drive, etc. She has had a traumatic past and benefits from staff support that can help her implement her coping strategies. She is described as a girly girl who likes to get her hair & nails done. She also loves to shop, watch movies, do crafts, and care for her pet bird. She enjoys cooking and learning steps to new recipes. There are no behavioral/mobility concerns currently. She can be shy and may need support to advocate for herself/learn new routines.
PROVIDER REQUIREMENTS: Provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Anxiety, Atypical Mood Disorder, Bipolar disorder, Expressive Language Disorder, Insomnia, Lactose Intolerant, Learning Disability, Major Depression, Moderate ID, Mood Disorder, Polycystic Ovarian Syndrome, PTSD, Traumatic Brain Injury.
HOUSING: Seeking new home in Lake County
DAY PROGRAM/EMPLOYMENT: Has job coaching currently. Works at multiple job sites independently with intermittent check-ins.
