DESCRIPTION of NEEDED SERVICES: This client is in need of HPC services every other weekend/1 evening shift per week.

DESCRIPTION of INDIVIDUAL: Client is an 84 yo female who lives with her Nephew/Provider in Concord, Ohio. She requires a wheelchair for mobility. She is very social and enjoys going out into the community.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Cerebral Palsy; Asthma; Spinal Stenosis; Arthritis.
HOUSING: She lives in Concord, Ohio with her Nephew.
DAY PROGRAM: N/A.
SUPPORTS NEEDED: HPC services with all ADL’s in her home as well as on outings in the community. 30-40 hours per week.
