DESCRIPTION OF REQUESTED SERVICES: This individual is a 36-year-old female who lives in Eastlake. She is looking to attend a day program with some vocational work 3-5 days per week. She may need verbal assistance with hygiene tasks, as well as reminders to use the restroom throughout the day. She also requires transportation in a non-modified vehicle.

DESCRIPTION OF INDIVIDUAL: She enjoys being social and likes to work! In years past, she worked independently in the community but has been at home for the past few years. Her long-term goal is to build up her skills to get back to independent employment but for now she is looking to have some work aspects built into the day, along with opportunities to socialize with her peers.
PROVIDER REQUIREMENTS: Requires reminders to use the restroom, verbal reminders with hygiene, and assistance with meal preparation.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Mild ID, Bipolar Disorder 2, schizophrenia, history of seizures, vitamin D deficiency, impaired fasting blood sugar, anxiety, lower back pain, L shoulder/neck strain, myopia, constant right eye esotropia, intradermal nervus benign. Allergies: seasonal
DAY PROGRAM/EMPLOYMENT: She does not currently attend a day program.
