DESCRIPTION OF REQUESTED SERVICES: 29-year-old male looking for a new program. He is interested in Voc. Hab or Group Employment. He is looking for 4 days a week (Monday-Thursday). He would need transportation to and from program.

DESCRIPTION OF INDIVIDUAL: He is verbal and speaks English. He may need to be encouraged to repeat himself slowly if you cannot understand him. Tends to speak fast and with a slur. He does not like loud noises and will avoid the area if he thinks it will be loud (dog barking, baby crying, etc.) He learns best through verbal instruction and modeling. Struggles with detail and needs to find tasks that he is interested in as he is easily distracted.
PROVIDER REQUIREMENTS: He is allergic to nuts and has a slight reaction to eggs. The reaction to eggs is less severe as he typically gets a rash when he eats them. The allergy to nuts requires usage of his epi-pen as he would react if he ate or touched a nut. He can sit at a separate table if another individual brings in a food with nuts.
TRANSPORTATION NEEDS: Non- modified
DIAGNOSES: He is diagnosed with Autism, Seasonal Allergies, Hashimoto’s Disease, and Anxiety.
HOUSING: He lives in Painesville with his family.
DAY PROGRAM/EMPLOYMENT: Currently looking for a new program.
