DESCRIPTION OF REQUESTED SERVICES: Individual is in need of NMT to work Monday-Friday. The individual needs picked up from his home, Painesville, OH at 9:30AM and taken to work in Willoughby, OH.

DESCRIPTION OF INDIVIDUAL: Individual is a 23-year-old male who lives in his family’s home in Painesville, OH near the Lake County Fairgrounds. He is verbal and requires no support for mobility. He does not like loud noises and will often listen to music during car rides. He has no history of behavior but thrives on routine.
PROVIDER REQUIREMENTS: NMT certified and willing and able to accommodate the time needed of the individual.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Autism Spectrum Disorder, Attention-deficit Hyperactivity Disorder (ADHD) and generalized anxiety disorder.
HOUSING: Lives with parents in Painesville, OH.
DAY PROGRAM/EMPLOYMENT: N/A
