DESCRIPTION OF REQUESTED SERVICES: 54-year-old female individual seeking a congregate setting 24-hour staffing with OSOC/HPC. She needs support to attend medical appointments, help make purchases, clean, meals and monitor her blood sugar/health needs.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Looking for agency with congregate home available. Open to sharing congregate/residential setting with female/male roommates.

DESCRIPTION OF INDIVIDUAL: This individual is kind, friendly, soft-spoken and values her independence. She enjoys playing games on her phone and her daily coffee. She enjoys exercising at Planet Fitness and Senior Center. This individual may want to attend a day program part time.
PROVIDER REQUIREMENTS: Provider would need to be able to deliver all ISP services as determined at ISP meeting.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Type 1 Diabetes, Osteoarthritis, Rheumatoid Arthritis, Kidney transplant recipient, Pancreas transplant recipient (unsuccessful and on the waiting list for another one), Esophageal Constriction, hypertrophic obstructive cardiomyopathy, coronary artery disease, GERD, asthma, hypertension, hypercalcemia, osteoporosis, non-intractable Epilepsy without status epilepticus, Anxiety, Schizophrenia, and Depression.
