DESCRIPTION OF REQUESTED SERVICES: The individual is a 37-year-old female who is diagnosed with a Cerebrovascular Accident and a tumor on her spine. Due to her conditions she has loss of strength in her limbs, and her neurologist has recommended a stairlift so she can access her bedroom and the full bathroom more easily. She lives in Eastlake with family.
