DESCRIPTION of NEEDED SERVICES: Individual would like Independent HPC provider to help with his ADL’s specifically showering 2x a week, help with making his bed and light cleaning, Tuesdays and Thursdays, 10:30am- 1:30am for a total 3-4 hours each day.

DESCRIPTION of INDIVIDUAL: Individual is a 33-year-old male living in Willoughby with his mom. He utilizes a specialized wheelchair and shower chair. He is able to support with transferring once he feels stable on his feet.
PROVIDER REQUIREMENTS: Independent HPC provider 2x weekly
DIAGNOSES: CP, Hearing Impaired
HOUSING: Individual lives at home with mom.
DAY PROGRAM: Does not attend.
SUPPORTS NEEDED: Individual needs assistance transferring from wheelchair onto shower chair he is weight bearing, most transfers he is independent.
