DESCRIPTION OF REQUESTED SERVICES: 27-year-old male individual looking for weekly HPC hours to be provided in the home and community. On average, about 4 hours weekly with transportation to and from planned activities.

DESCRIPTION OF INDIVIDUAL: 27-year-old male individual currently living in Perry with his father. They moved from Arizona in November of 2024 and are working to connect to the community/increase the individuals’ daily activities. This client loves all things nature! The more to do outside, the better. He enjoys the waves, peaceful calm environments, and going to Buffalo wild wings. He knows how to horseback ride and really loves to be with animals. He loves music (especially Christmas songs, songs from Barney/his favorite shows). He enjoys jumping on trampolines, gymnastics, and bouncing on an exercise ball. Over the last year or so he has had a mental health decline and an increase in verbal outbursts/aggression. Typically, he will grab the arm or shoulder of someone and squeeze tightly.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Autism, Encephalitis, Impulse Control Disorder.
HOUSING: Lives at home in Perry with his father.
DAY PROGRAM/EMPLOYMENT: Attends CLW 3 days per week (may increase to 5 days shortly)
