DESCRIPTION OF INDIVIDUAL: Individual is a 42-year-old female. Individual enjoys making money and using her money for things she enjoys. She enjoys having nice nails, buying clothes, and Disney themed things. She also has two small dogs and enjoys caring for them. Individual needs encouragement to make healthy choices for her diabetes. She needs support to learn expectations for community employment as this is her ultimate goal (ex: work appropriate clothing, good hygiene, caring for health needs, and regular work attendance). She receives prompts to check her work for thoroughness. Positive feedback helps her. She wants to make minimum wage and have consistent job tasks through her entire shifts. She reports that she can be easily distracted and prefers to have set daily expectations and goals.
PROVIDER REQUIREMENTS: GES or VH Certification
TRANSPORTATION NEEDS: Will receive NMT services from current provider.
DIAGNOSES: frequent UTIs, diabetes, lactose intolerance, migraine headaches, restrictive lung disease. She has been diagnosed with Schizoaffective Disorder, Borderline Personality Disorder, developmental delays, bipolar disorder, Depression, and Arthritis.
HOUSING: Lives in Willowick in group home.
DAY PROGRAM/EMPLOYMENT: Looking for new GES or VH provider.
