DESCRIPTION OF REQUESTED SERVICES: Individual would like Independent HPC provider to help with his ADL’s specifically showering 2x a week, help with making his bed and light cleaning, Tuesdays and Thursdays, 10:30am- 1:30pm for a total 3-4 hours each day.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Independent Providers only. No preference on male/female. Individual has had occurrences of using racial slurs.

DESCRIPTION OF INDIVIDUAL: Individual is a 33-year-old male living in Willoughby with his mom. He utilizes a specialized wheelchair and shower chair. He is able to support with transferring once he feels stable on his feet. Individual is hearing impaired and reads lips as well as requests clear speech from provider where he can see their face. Individual feels strongly about timeliness and call offs. It is pivotal to long term success with individual to be on time, stick to the days and times he has requested service (not change based on providers needs/schedule), and to have appropriate communication when emergencies require a cancelation/call off. Individual will communicate via text, no calls due to hearing impairment.
PROVIDER REQUIREMENTS: Independent Provider Certification
TRANSPORTATION NEEDS: Not needed.
DIAGNOSES: CP, Hearing Impaired
HOUSING: Willoughby, OH lives with family.
