DESCRIPTION OF REQUESTED SERVICES: Individual is seeking HPC services to increase his engagement in community activities. Individual is seeking HPC services roughly 2 days a week, days/hours are flexible.

DESCRIPTION OF INDIVIDUAL: Individual is a 31-year-old male residing in Willoughby in his family home. Individual is verbal, but may be difficult to understand at times, especially when excited. This individual is friendly, caring, outgoing, and follows directions well. Individual can make his own day-to-day decisions regarding what he wants to do, where he wants to go, and what he wants to eat but may require assistance from his guardian or staff to make more complex decisions. He enjoys going to the movies, parks, participating in sports, going out to eat and shopping. Individual can occasionally lose control of his behavior. He has trouble letting things go and can make small issues into big ones. He benefits from being able to talk/ vent to others when he is upset or anxious about something. Individual benefits from general supervision while in the community.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Moderate ID, Autism/ PDD NOS, Tourette’s Syndrome, ADD with Hyperactivity, Asthma, Anxiety, OSA (Apnea), Allergic Rhinitis, Scoliosis, Dysphonia, PTSD, Eczema, Hiatal Hernia, Epilepsy, and Skin Cancer.
DAY PROGRAM/EMPLOYMENT: No current day Program or Employment services.
