DESCRIPTION OF REQUESTED SERVICES: 26-year-old female individual looking for a Waiver Home opening in Lake County. Prefers to be in a home with female housemates. HPC/OSOC hours required.

SEEKING AGENCY OR INDEPENDENT PROVIDER: Seeking Agency provider with current waiver home openings.

FUNDING SOURCE: Level One

DESCRIPTION OF INDIVIDUAL: This individual can be described as organized, social, and outgoing. She enjoys taking care of and supporting others. She is an effective communicator and can directly express her wants and needs. She is her own guardian and has family support in place. She enjoys getting her nails done, playing basketball, and spending time with animals. She is hopeful to find a home with roommates that have similar interests to her. She is independent with her self-care tasks/ADL’s/can independently administer her medications but needs help to ensure that they are filled. She enjoys spending money and will need support to ensure her budget is considered and bills are managed. She likes to know about any changes as much as possible to avoid/reduce anxiety. She can manage 12 hours of alone time in her home with daily check-ins via phone and up to 4 hours of alone time in the community if in a familiar location. She needs physical assistance to schedule & attend medical appointments.

PROVIDER REQUIREMENTS: Provider would need to be able to provide services as identified in ISP.

TRANSPORTATION NEEDS: Non-Modified vehicle. Able to take public transportation.

DIAGNOSES: Autism Spectrum Disorder, TIC Disorder, Asthma and ADHD. May be sensitive to medications

HOUSING: Seeking waiver home Lake County.

DAY PROGRAM/EMPLOYMENT: Hopeful to secure employment once housing is secured.
