DESCRIPTION OF INDIVIDUAL: 1. Female individual, verbal, very social, loves to go out into the community and very crafty.
- Male individual, verbal, Soft spoken, very kind and enjoys trips out into the community or watching tv.
PROVIDER REQUIREMENTS: Med certification 1
TRANSPORTATION NEEDS: non modified vehicle.
DIAGNOSES: 1. Mild ID, Bipolar disorder NOS, Anxiety disorder, ADHD, High blood pressure, Athletes Foot, reoccurring cellulitis.
- noninsulin Dependent Diabetes, constipation, Ambulation Problems, Heart Disease, Asthma, Hypertension, Partial Complex Seizures, and Moderate Intellectual Disability.
HOUSING: At their home in Wickliffe
