DESCRIPTION of INDIVIDUAL: 26 year old male. He Enjoys watching movies, playing video games, puzzles, dancing, listening to music and watching YouTube. Individual is easy going and gets along well others.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Pervasive Developmental Disorder
DAY PROGRAM: Currently attends program 5 days and does not need transportation
SUPPORTS NEEDED: Individual needs assistance with meal preparation, verbal reminders to complete hygiene care and complete laundry. Does well with routine and structure.
