DESCRIPTION OF REQUESTED SERVICES: Individual is seeking a provider for round trip non-medical transportation (NMT) to get to/from home and work. Home is located on Bates Road in Madison (between East Main Street and Middle Ridge Road) and work is located off Plaza Boulevard in Mentor (between Tyler Blvd and Mentor Avenue). He works on Monday and Wednesday from 9:00am – 2:00pm.

HOUSING: Resides with family in Madison, Ohio
DAY PROGRAM/EMPLOYMENT: Beginning new part-time employment on March 16th. He’ll be working M/W from 9am-2pm and seeking NMT provider to get him to/from work.
