DESCRIPTION OF INDIVIDUAL: This individual is a 71-year-old living with spouse in a home in Mentor-on-the-Lake. Due to her mobility declining recently, this individual has fallen several times within the last year. To enhance her safety, especially when she is home alone and does not have her phone within reach, this individual would like something that she is able to wear to alert emergency responders that she needs assistance. This individual is verbal and is able to use a cell phone. She uses a walker but will sometimes use it less at home and is unsteady. This individual will attempt to move quickly and is not always aware of surroundings, causing her to be at risk of falling or losing her balance.
PROVIDER REQUIREMENTS: Certified to obtain, install, repair, and train individual on use of equipment.
TRANSPORTATION NEEDS: N/A
DIAGNOSES: Mild Intellectual Disability, sensory loss in right foot, Depression, right leg shorter, GERD, IBS, Cephalgia (headaches), High Blood Pressure, Incontinence, Sleep Apnea, High Cholesterol and history of breast cancer Stage 1 on the right side.
HOUSING: Individual lives in a home in Mentor-on-the-Lake
DAY PROGRAM/EMPLOYMENT: Individual attends a day program 2 days per week
