DESCRIPTION OF REQUESTED SERVICES: Individual is in need of NMT provide 10 trips weekly from home in Madison to program in Euclid.

DESCRIPTION OF INDIVIDUAL: Individual is a 22- years old male. He needs support with communication as he is non-speaking. Individual can exhibit self-injurious behavior when agitated or overstimulated. He can become aggressive with loud sudden noises.
PROVIDER REQUIREMENTS: Crisis intervention training is recommended.
TRANSPORTATION NEEDS: Non-modified vehicle. Individual currently sit at the rear of van and do not ride with other passengers in the rear.
DIAGNOSES: Severe Autism, nonverbal with aggression, Autistic Catatonia, leaky gut syndrome, Hypotonia, seizures, and history of insomnia. Individual avoids gluten.
HOUSING: Lives in Madison with family.
DAY PROGRAM/EMPLOYMENT: Attends Oghosa ADS, in Euclid 5 days a week.
