DESCRIPTION OF INDIVIDUAL: This individual is a 32-year-old happy, social and friendly male with a diagnosis of Failure to Thrive; Postnatal. He is verbal and resides with his mother & father in a family home in Willoughby. He is very outgoing and loves to be around others and stay busy.
PROVIDER REQUIREMENTS: Follow ISP language.
TRANSPORTATION NEEDS: Non-Modified Vehicle
DIAGNOSES: Failure to Thrive, Postnatal
HOUSING: Individual lives with his parents in Willoughby
DAY PROGRAM/EMPLOYMENT: CES (works when medically feeling ok)
