DESCRIPTION OF REQUESTED SERVICES: The individual is a 77-year-old female who has mobility deficits due to progressive osteoporosis. She lives in a congregate setting in Perry. Currently there is a long concrete ramp at the front door but in the garage, there are 2 steps from the house. Looking for a provider who can widen the garage door from the house and install a ramp so that the individual does not need to go outside to get to the van during inclement weather.
