DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a vocational habilitation program to attend 2-3 days per week; likely on Mondays, Tuesdays and Wednesdays. This person would also need transportation to and from the day program in a non-modified vehicle. This individual enjoys going out into the community as well.

DESCRIPTION OF INDIVIDUAL: Individual is a 44-year-old male. This individual enjoys cutting up cardboard or writing on cardboard. He loves cats and shopping at thrift shops. This individual can become easily distracted and sometimes needs reminders to stay on task but is able to complete tasks independently once tasks are modeled for him.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: This individual is diagnosed with a mild intellectual disability, attention deficit disorder, hard of hearing and tone deaf, bronchia pulmonary dysplasia, thyroid nodule and goiter.
HOUSING: This individual lives with family in Eastlake
DAY PROGRAM/EMPLOYMENT: This individual is currently employed 2 days per week with job coaching and is looking for additional opportunities to earn money while interacting with his peers and going out into the community on outings.
