DESCRIPTION OF REQUESTED SERVICES: Individual is a 58-year-old male who is currently seeking HPC services in a congregate setting in the community. This individual needs visual supervision in the community and auditory supervision in the home and awake staff at night. Individual needs to be monitored during meals due to choking risk. Individual has limited verbal skills, but can communicate by writing words/phrases and body gestures. Individual requires mostly total assistance complete all ADLs and household cleaning tasks. He enjoys shopping, going out to eat, watching TV and football. He is very smart, kind, helpful and has a great memory.

DESCRIPTION OF INDIVIDUAL: Individual has Spastic Quadriplegia and is dependent on his wheelchair for mobility. He has a Baclofen Pump and utilizes a urinal during the day and a Texas Catheter at night. He is on a pureed diet with thickened liquids. Individual has a history of frequent UTIs.
PROVIDER REQUIREMENTS: Medication cert I and II
TRANSPORTATION NEEDS: Individual needs a modified vehicle due to having a wheelchair.
DIAGNOSES: Cerebral palsy w/ spastic quadriplegia, borderline MR w/ severe adaptive deficits, allergic rhinitis, ceruminosis, constipation, dyspnea, decubitis ulcer
DAY PROGRAM/EMPLOYMENT: Attends a day program 3 days/week. Program provides transportation
