DESCRIPTION OF REQUESTED SERVICES: 32-year-old female is looking for a NMT driver to take her to work Tuesday, Wednesday and Thursday. Preferably a female. She needs to be at Ballantine’s in downtown Willoughby by 8:30.

TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Mild ID, Developmental Delay, Anxiety, Mild CP, and Hydrocephalus secondary to Meningitis with VP Shunt, Legally Blind L eye, Near & Far Sighted
HOUSING: Lives with her mom in Concord.
DAY PROGRAM/EMPLOYMENT: Works through CES at Ballantine’s
