DESCRIPTION OF INDIVIDUAL: Individual is a 21-year-old male. He lives in Eastlake. He is active with independent work and special Olympics. He is in need of social and fun activities a couple of days a week. He is Autistic and has some allergies. His allergies are not life threatening but require avoidance. He loves to be active and busy.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-Modified transportation
DIAGNOSES: Autism
HOUSING: Individual lives with parents in Eastlake.
DAY PROGRAM/EMPLOYMENT: He attends Spark day program and works at Giant Eagle
